Please complete this form to help you create a contract.  Note: Students are not authorized to sign contracts.   You should rely on either Maribeth Overland (x5273) or Gwen Schimek in Student Activities to create a legal contract for you.  

Note: Before you contract anything ( DJ, Civic center, band, speaker, etc.), contact Department of Student Activities and Leadership Development for help! We can make the process easier for you and ensure that you are legally protected.  Even if an artist is speaking or performing for free, you should still have the agreement in the form of a contract.

Students put themselves at significant risk for liability issues when they hire an outside party without a contract signed by a Student Activities staff member.

Is the artist from MN, WI, MI, ND, SD?  If not, inform agent that 2% will be deducted from artist fee to pay Minnesota Entertainment tax.  CSB|SJU does not pay a deposit and will pay artist by University check following the performance.  Make sure that they know that the contract is for both SJU and CSB. 

You should request that the artist/agent send the following insurance information to Student Activities one month in advance of the performance:

           1) Certificate for $1,000,000 general liability
           2) Certificate confirming auto liability
           3) Certificate confirming worker’s compensation for all artists/performers/etc.

We CANNOT make any advance payments or deposits.

What are your responsibilities as the event sponsor?

As the event sponsor you should:

_____   Fill out the worksheet above and forward to Student Activities, Mary Commons 208, CSB.   Student Activities will complete the necessary contract and other paperwork, and work directly with the artist/performer to get the paperwork signed.

_____    Confirm that the performance space is reserved

_____    Make necessary hotel or room reservations

_____   Consider arrangements for any additional hospitality (food, transportation, etc)

_____   Complete an Event Notification Form; this will advertise your event on the Student Calendar        

_____    Pick up the following paperwork from Student Activities when notified:

  • the signed contract               
  • the attached riders
  • the signed W-9
  • the artist’s insurance information

 _____    Complete a check request for the amount you owe the artist* and take the check request --  along with a copy of all the paper work listed above -- to the Business Office.  Payment is the responsibility of the event sponsor, not Student Activities

_____    Finalize all hotel, travel and hospitality arrangements

 _____    Arrange for any media or other special needs

 _____    Pick up the check at the Business Office 

 _____    Have a successful event!
